Hospital and care home notary
We come to hospitals, rehab centers, hospices, nursing homes and assisted living. The signer must be able to show a valid ID (or be identified another legal way) and sign in front of us. A $25 facility fee applies, plus the travel fee for your town.

Before you book a hospital visit
- Check the signer can take part. They must be able to understand what they are signing and sign in front of the notary. If they cannot write their name, ask us before booking.
- Find the ID. Hospitals often keep patients' belongings away from the room. Make sure a current ID is at hand.
- Ask the facility about visitors. Some units limit visiting times or need a visitor badge.
- Have the document complete and unsigned. Every blank filled in before we arrive.
If the signer has no ID with them

- Where a signer has no acceptable ID, identity can be proven by the sworn written statement of one credible witness the notary personally knows, or two credible witnesses who prove their own identity — with no financial interest in the transaction.Florida Statutes s. 117.05(5)(b)1, read September 28, 2026

- A notary may not take an acknowledgment or oath from a person they know a court has declared mentally incapacitated, where the right involved has been removed and not restored.Florida Statutes s. 117.107(4), read September 28, 2026

We cannot judge a person's medical capacity, and we cannot notarize if the signer does not appear to understand or agree. If there is any doubt, the family may want to speak with an attorney first.

Can a patient sign if they are in bed?
Yes, as long as they can take part and sign in front of the notary.
Can a relative sign for the patient?
Only if they have the legal authority to do so, such as a valid power of attorney. A notary cannot decide that — ask an attorney if you are unsure.
